Enrollment can be completed by following the “Enroll Now” link on the homepage during the enrollment period of August 15-September 15.
Important note:
Before you begin the enrollment process, be sure that you have all the information necessary to complete the enrollment forms including your complete address and the names and dates of birth of all dependents that are to be enrolled.
Once you have selected and paid through your chosen payment option, your enrollment will be finalized and effective from October 1, 2024. If you do not complete the enrollment process for any reason, you will be allowed to re-enter the enrollment website to finish your enrollment. However, after payment is made for either or both the Delta Dental and EyeMed plans, your enrollment is final, and you will not be able to cancel or add any plans.
In other words, if you complete your enrollment and payment for the Delta Dental plan but do not enroll in EyeMed, you will not have another chance to enroll in EyeMed. Similarly, if you complete your enrollment and payment for the EyeMed plan but do not enroll in Delta Dental, you will not have another chance to enroll in Delta Dental.
Graduate & Professional students
You should have a major credit card (Visa, MasterCard, Discover or American Express) with you to pay for the plan(s), or if applicable charge to your student account, in order to complete the enrollment process.
To complete the enrollment process and be enrolled in either or both the Delta Dental and EyeMed plans, you must pay the plan cost online.
PhD students
To complete the enrollment process and be enrolled in either or both the Delta Dental and EyeMed plans, you must choose either payroll deduction, major credit card (Visa, MasterCard, Discover or American Express), or if applicable charge to your student account. Once you have selected and paid through your chosen payment option, your enrollment will be finalized and effective from October 1, 2024.

Yes. You must first enroll in the Yale Charge Account Plan. This is done through YaleHub SIS from the Student Accounts tab - Charge Account Authorization. Please note, however, that if items charged to the Student Account are not paid on time, late fees may apply.
Coverage becomes effective on October 1, 2024. It will last through September 30th, 2025.
You may enroll your lawfully married spouse or civil union partner and/or legally dependent children who are under the age of twenty-seven as of October 1, 2024.
Visit www.deltadentalct.com and use Delta Dental’s dentist search to find a participating dentist near your home or campus. Two networks are available: Delta Dental PPOSM and Delta Dental Premier®. The Delta Dental PPOSM network is a smaller, more deeply discounted network. You will maximize your benefits with these lower cost providers. Delta Dental Premier® is a larger network with more modest discounts. Approximately 80% of all dentists participate in the Delta Dental Premier® network and approximately 45% participate in the Delta Dental PPOSM network. Your plan offers enhanced benefits with Delta Dental PPOSM dentists.
Use Delta Dental’s dentist search to find a participating PPO network dentist near your home or campus. We recommend that you call your dentist to determine the cost of your specific procedure. When you do that, be sure to tell your dentist that you are a member of the Delta Dental of Connecticut plan for Yale Graduate and Professional School Students, or PhD students, not the Yale employee plan. On or after October 1, 2024, go online to get your Delta Dental membership ID card. Visit Delta Dental CT website. Login/register for an account using your UPI number found on your Yale ID card. Click the ID card icon. Your ID card will display, and you can print it immediately.
